Request For Donation Form

J&J Nursery and Garden Center allocates funds for donations through their annual set advertising budget. Due to the number of requests received, it has become necessary to establish this donation policy. A Request Form for funds/donations /sponsors must be filled out and returned to J&J Nursery by way of email, fax (see above) or in person. The form/documents will be reviewed by the Owner’s. You will receive notification upon their decision.
